A Senior Care Physician is needed to join an established team serving PALTC patients in community living facilities. The role focuses on building patient relationships and delivering high-quality care.

Position Details

  • Full-time (4x10) and part-time options with flexible schedules
  • Team of 5 physicians and 10 APCs
  • Visit 2–3 skilled nursing facilities daily; patient schedules are built in EPIC
  • Phone-only call rotation: 1:11
  • Four weekends per year; physicians cover one holiday per year
  • Weekly backup call is available to support APCs if needed; no in-person call visits
  • Patient census of 8–12
  • RNs and schedulers coordinate provider visits, communicate with facilities, and triage phone calls
  • 27 skilled nursing and assisted-living facilities served
