Description

Provide routine primary care and follow-up for patients in nursing home and subacute facilities. Duties include physical assessments, diagnosis and treatment of general practice conditions, chronic disease management, documentation, ordering and interpreting tests and imaging, immunizations, and patient counseling. You will coordinate with facility staff and collaborate with supervising physicians and the care team.

Responsibilities

• Examine, diagnose, and treat patients

• Collect, record, and maintain accurate patient information

• Document past medical, family, and social history; review of systems; exams; medications; allergies; assessment and plan

• Order or perform tests, analyses, and diagnostic imaging; analyze findings and reports to make diagnoses

• Administer or prescribe treatments

• Order lab and imaging tests to determine and manage an immediate treatment plan

• Vaccinate patients to prevent communicable diseases

• Advise patients on diet, hygiene, and methods to prevent disease

• Consult specialists when necessary and refer patients within the network

• Provide care to patients in nursing homes

Requirements

• Master’s degree in a Nurse Practitioner program (Family or Adult Geriatrics)

• Current New York State Nurse Practitioner license

• National Board Certification required (AANP, ANCC, or ONCC)

• Bilingual (Spanish speaking) is a plus

• EMR/EHR experience is a plus
