Description

You will provide advanced primary and/or specialty care to patients, including performing physical exams, recording medical history, creating care plans, and administering diagnostic tests.

Responsibilities

• Examine, diagnose, and treat patients
• Collect, record, and maintain accurate patient information
• Document medical history (past, family, social), review of systems, exams, medications, allergies, assessment, and plan
• Order or perform tests, analyses, and diagnostic imaging; analyze findings to diagnose conditions
• Administer or prescribe treatments and manage immediate treatment plans
• Order lab and imaging tests
• Vaccinate patients
• Advise patients on diet, hygiene, and disease prevention
• Consult appropriate specialists and refer within the network as needed
• Provide care to patients in nursing homes

Requirements

• Master’s Degree in a Nurse Practitioner program: Family or Adult Geriatrics
• Current New York State license as a Nurse Practitioner
• National Board Certification (AANP, ANCC, or ONCC)
• Bilingual (Spanish speaking) is a plus
• EMR/EHR experience is a plus
