Description

Provide high-quality, patient-centered care during home visits, with a focus on holistic management of complex medical conditions. You will complete comprehensive assessments, diagnose and treat conditions, educate patients and families, coordinate care with other providers, and maintain documentation in electronic health records. The role includes follow-up care and responding to urgent medical issues in the home setting.

Responsibilities

• Conduct comprehensive patient evaluations (physical exams, medical history review, medication reconciliation)
• Diagnose conditions and develop individualized treatment plans; prescribe medications as needed
• Coordinate care with physicians, specialists, and other healthcare providers to ensure continuity
• Provide patient and family education on health management, lifestyle modifications, disease prevention, and medication adherence
• Maintain accurate documentation of assessments, treatments, and follow-up in electronic health records
• Schedule and conduct follow-up visits to monitor progress and adjust treatment plans
• Assess and manage urgent medical issues during home visits; provide referrals or interventions as needed
• Participate in outreach efforts to promote health services and educate the community on available resources

Requirements

• Master’s degree in a Nurse Practitioner program: Family or Adult Geriatrics
• Current New York State license as a Nurse Practitioner
• National Board certification required: AANP, ANCC, or ONCC
• Ability to provide care primarily in patients’ homes, including varied living conditions
• Travel within a designated geographic area and flexible schedule (possible evenings and weekends)
