Description

Provide advanced primary and/or specialty care to patients, including physical exams, documenting medical history, creating care plans, and administering diagnostic testing. Examine, diagnose, and treat patients across general practice, maintain accurate patient records, and order or execute tests to support diagnoses and treatment. Provide preventive care (including vaccinations and counseling), coordinate with specialists as needed, and deliver care to nursing home patients.

Responsibilities

• Examines, diagnoses, and treats patients

• Ensures quality-care commitments are met

• Diagnoses and treats a variety of diseases in general practice

• Collects, records, and maintains accurate patient information

• Documents past medical/family/social history, review of systems, examinations, medications, allergies, assessment, and plan

• Orders or executes diagnostic tests, analyses, and diagnostic imaging

• Analyzes test reports and examination findings to diagnose conditions

• Administers or prescribes treatments

• Orders lab and imaging tests to determine and manage an immediate treatment plan

• Vaccinates patients

• Advises patients on diet, hygiene, and disease-prevention methods

• Consults with appropriate medical specialists and refers within the network

• Provides care to patients in a nursing home

Requirements

• Master’s Degree in a Nurse Practitioner Program: Family or Adult Geriatrics

• Current New York State license as a Nurse Practitioner

• National Board Certification required (AANP, ANCC, or ONCC)

• Bilingual (Spanish-speaking) is a plus

• EMR/EHR experience is a plus
