Description

You will provide continuum quality routine patient care and rehabilitation for nursing home residents, including examination, diagnosis, treatment, and ongoing documentation. The role also supports preventive care (e.g., vaccinations) and patient counseling, with referrals to specialists when necessary.

Responsibilities

• Examine, diagnose, and treat patients
• Ensure fulfillment of the organization’s commitment to quality care
• Diagnose and treat a variety of diseases in general practice
• Examine patients using medical instruments and equipment
• Collect, record, and maintain accurate patient information
• Document past medical, family, and social history; review of systems; examinations; medications; allergies; assessment and plan
• Order or execute tests, analyses, and diagnostic imaging
• Analyze test and examination reports to diagnose patients’ conditions
• Administer or prescribe treatments
• Order lab and imaging tests to determine and manage an immediate treatment plan
• Vaccinate patients to immunize against communicable diseases
• Advise patients on diet, hygiene, and methods for prevention of disease
• Consult appropriate specialists and refer patients within the network

Requirements

• Medical degree conferred by an accredited college or university
• New York State licensure
• Board Eligible and/or Board Certified
• Demonstrated clinical excellence
• Nursing home experience strongly preferred
