Description

Provide primary medical care services to nursing home patients in line with the standard of care, working with the primary healthcare team.

Responsibilities

• Order laboratory and radiology tests and refer patients to specialists when appropriate

• Analyze reports, test results, medical records, and examinations to diagnose conditions and propose treatment options

• Prescribe pharmaceuticals/other medications and treatment regimens for documented conditions

• Review and discuss potential side effects with patients and keep allergy information up to date before prescribing

• Advise patients on lifestyle and diet changes that may improve health or treat health issues

• Adhere to departmental policies, quality improvement objectives, and safety/environmental/infection-control standards

• Maintain patient confidentiality and comply with federal and state health information privacy laws

• Record complete, timely, and legible medical records

Requirements

• BC/BE in Geriatrics, Internal Medicine, Palliative Care/Hospice, or Family Medicine

• MD or DO degree from an accredited U.S. medical school

• Successful completion of residency (or completed U.S. residency / 5-year pathway)

• Active New York medical practitioner license

• Current DEA and Department of Public Safety (DPS) registration for prescription writing

• Experience providing primary care and diagnostic services in a clinical setting

• Experience working in a team of healthcare professionals and collaborating to ensure effective patient experiences
