Description

Provide routine primary care and rehabilitation in general practice, ensuring high-quality continuum care. You will examine, diagnose, and treat patients, order and interpret tests and diagnostic imaging, and manage treatment plans. You will also document patient histories and findings, vaccinate patients, and refer to specialists when needed.

Responsibilities

• Examine, diagnose, and treat patients

• Ensure quality of care standards are met

• Diagnose and treat a variety of general practice conditions

• Collect, record, and maintain accurate patient information

• Document past medical, family, and social history; review of systems; examinations; medications; allergies; assessment and plan

• Order/execute tests, analyses, and diagnostic imaging; analyze reports and findings

• Administer or prescribe treatments; manage immediate treatment plans

• Vaccinate patients to prevent communicable diseases

• Advise patients on diet, hygiene, and disease prevention

• Consult with medical specialists and refer within the network when necessary

Requirements

• Medical degree from an accredited college or university

• New York State licensure

• Board Eligible and/or Certified

• Demonstrated clinical excellence

• Nursing Home experience strongly preferred
