Description

Provide comprehensive assessments, physical exams, and individualized care plans while managing both acute and non-acute conditions. Work independently and with a multidisciplinary team, participate in clinical rounds, and support quality management programs. Act as a patient advocate by facilitating family conferences and ensuring patient education on care plans.

Requirements

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar

• Active state licensure

• Board certification preferred

• Minimum 2 years in a clinical setting

• Proven ability to manage a patient panel

• Leadership experience preferred

• Geriatric care experience is a plus
