Description

The Physician Assistant specializing in house calls provides high-quality, patient-centered medical care to individuals in their homes. The role emphasizes effective communication, collaboration with the healthcare team, and continuous care for patients with chronic illnesses or mobility/access barriers. You will assess, diagnose, treat, educate, document in EHRs, and manage follow-ups and urgent needs.

Responsibilities

• Perform thorough patient evaluations during home visits (physical exams, health histories, medication reviews)
• Diagnose conditions and develop personalized treatment plans; prescribe medications and therapies as appropriate
• Collaborate with physicians, nurses, and other team members to ensure comprehensive, continuous care
• Educate patients and families on disease management, medication adherence, preventive care, and lifestyle changes
• Maintain accurate and detailed encounter and treatment documentation in electronic health records
• Schedule and conduct follow-up visits; adjust treatment plans based on patient progress
• Assess and address urgent medical needs; coordinate referrals and additional support as required
• Participate in community outreach initiatives to promote awareness of home healthcare services and resources

Requirements

• Master’s Degree required
• Active New York Physician Assistant license
• Current New York controlled substance and DEA registration
• Graduate of an approved PA program
• Post-graduate training in surgery OR at least one-year equivalent work experience
• Current certification by the National Commission on Certification of Physician Assistants (NCCPA)
• Emergency Medicine or Family Practice or Acute Care experience
• DOT, BLS, ACLS preferred
• Bilingual Spanish a plus
• Proven work experience as a physician assistant or nurse
• Knowledge of EMR systems preferred
