Description

Provide high-quality, patient-centered care through home visits for people with chronic illnesses, mobility challenges, or limited access to healthcare facilities. Focus on holistic care, patient education, and management of complex medical conditions to maintain continuity of care in patients’ homes.

Responsibilities

• Perform comprehensive patient evaluations during home visits (physical exams, medication reconciliation, medical history review).
• Diagnose conditions, develop individualized care plans, and prescribe medications as appropriate.
• Coordinate care with physicians, specialists, and other healthcare professionals.
• Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive care.
• Document assessments, treatments, and follow-ups in detailed, accurate EHR records.
• Conduct follow-up visits to monitor progress and adjust treatment plans.
• Assess and manage urgent medical situations during visits, including timely interventions or referrals.
• Join local outreach initiatives to promote available health services and community resources.

Requirements

• Master’s degree in a Nurse Practitioner program (Family or Adult Geriatrics).
• Current Massachusetts Nurse Practitioner license.
• National Board Certification (AANP, ANCC, or ONCC).
• EMR/EHR experience preferred; new graduates encouraged.
• Bilingual (Spanish-speaking) is a plus.
• Ability to travel within Massachusetts and provide care primarily in patients’ homes with flexible scheduling.
