Description

Provide patient-centered primary care through home visits in the Greater Boston region for individuals with chronic illnesses, mobility challenges, or limited access to healthcare facilities. Emphasize holistic care, patient education, and management of complex medical conditions to ensure continuity of care in patients’ homes.

Responsibilities

• Perform comprehensive health assessments during home visits (physical exams, medication reconciliation, and medical history review).
• Diagnose conditions, create individualized care plans, and prescribe medications as appropriate.
• Coordinate care with physicians, specialists, and other healthcare professionals.
• Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive care.
• Document assessments, treatments, and follow-ups in the electronic health record (EHR).
• Conduct follow-up visits to monitor progress and adjust treatment plans.
• Assess and manage urgent medical situations during home visits, including timely interventions or referrals.
• Participate in local outreach initiatives to promote available health services and community resources.

Requirements

• Master’s degree in a Nurse Practitioner program (Family or Adult Geriatrics).
• Current Massachusetts Nurse Practitioner license.
• National Board Certification via AANP, ANCC, or ONCC.
• Ability to provide care primarily in patients’ homes; variable living conditions.
• Travel within Massachusetts and flexible scheduling, including possible evenings and weekends.
• Bilingual Spanish is a plus.
• EMR/EHR experience preferred; new graduates encouraged.

Work Environment

• Home-based care within the Greater Boston region.
