Description

Provide advanced primary and/or specialty care to patients through home visits. You will perform exams, document history and care plans, order and interpret diagnostic tests, prescribe treatments, and coordinate referrals when specialist input is needed.

Responsibilities

• Examine, diagnose, and treat patients

• Collect, record, and maintain accurate patient information

• Document past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan

• Order or execute tests, analyses, and diagnostic imaging

• Analyze reports and exam findings to diagnose conditions

• Administer or prescribe treatments

• Order lab and imaging tests to determine and manage an immediate treatment plan

• Vaccinate patients

• Advise patients on diet, hygiene, and disease prevention

• Consult specialists when necessary; refer patients within the network

• Provide care to patients in their homes

Requirements

• Master’s degree in a Nurse Practitioner Program (Family or Adult Geriatrics)

• Current New York State license as a Nurse Practitioner

• National Board Certification (AANP or ANCC)

• Ability to provide care during home visits

Benefits
