Description

Provide virtual clinical oversight for patients receiving in-home care, including telehealth visits, assessments, and care coordination. You will manage complex chronic conditions, support medication management, and collaborate with field-based clinicians and care teams. Document clinical decisions and ensure compliance with clinical protocols, quality standards, and regulatory requirements.

Responsibilities

• Conduct telehealth visits for patients enrolled in the House Calls program
• Manage chronic conditions including diabetes, hypertension, CHF, COPD, and geriatric syndromes (when primary care is required)
• Perform care management needs assessments, follow-ups, and care plan adjustments
• Review patient histories, labs, imaging, and diagnostic results
• Support medication management and reconciliation
• Coordinate care with primary care providers (if applicable), specialists, and care management teams
• Document visits and clinical decisions accurately in the EMR
• Ensure compliance with clinical protocols, quality standards, and regulatory requirements
• Participate in interdisciplinary case reviews and quality improvement initiatives

Requirements

• Active NP license in NY and one or more of: NJ, PA, OH, NE, TX (or willingness to obtain)
• National NP certification (ANCC or AANP)
• Minimum 2+ years of clinical experience as a Nurse Practitioner
• DEA registration (active or willingness to obtain, if applicable)
• Comfort managing complex patients remotely and making strong clinical judgments
• Excellent communication and documentation skills
• Experience in house calls, home health, geriatrics, primary care, or internal medicine (preferred)
• Prior telehealth or remote care experience (preferred)
