Description

Provide comprehensive home visits for patients with complex and chronic conditions, emphasizing holistic care, patient education, and continuity. Conduct physical exams and medication reconciliation, diagnose conditions, create individualized care plans, and coordinate with physicians, specialists, and other providers. Maintain detailed EHR documentation, monitor progress through follow-up visits, and assess/manage urgent issues during home visits with timely referrals when needed.

Responsibilities

• Conduct comprehensive health assessments during home visits (physical exams, medication reconciliation, review of medical history)

• Diagnose medical conditions, develop individualized care plans, and prescribe medications as appropriate

• Coordinate care with physicians, specialists, and other healthcare professionals

• Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive care

• Maintain accurate electronic health records (EHR) of assessments, treatments, and follow-ups

• Provide follow-up care and adjust treatment plans based on patient progress

• Assess and manage urgent medical situations during home visits; intervene or refer promptly

• Participate in local outreach initiatives to promote health services and community resources

Requirements

• Master’s Degree in a Nurse Practitioner program (Family or Adult Geriatrics)

• Current Massachusetts Nurse Practitioner license

• National Board Certification through AANP, ANCC, or ONCC

• Bilingual Spanish is a plus

• Experience with EMR/EHR systems preferred
