Description

Provide telehealth-based clinical oversight for patients enrolled in an in-home primary care House Calls program. Conduct virtual patient assessments, manage chronic conditions, coordinate care with providers and care management teams, and support medication reconciliation. Ensure accurate documentation in the EMR and compliance with clinical protocols, quality standards, and regulatory requirements.

Responsibilities

• Conduct telehealth visits for House Calls program patients

• Manage chronic conditions including diabetes, hypertension, CHF, COPD, and geriatric syndromes (when the patient requires primary care)

• Perform care management needs assessments, follow-ups, and care plan adjustments

• Review patient histories, labs, imaging, and diagnostic results

• Support medication management and reconciliation

• Coordinate care with primary care providers (if applicable), specialists, and care management teams

• Document visits and clinical decisions accurately in the EMR

• Ensure compliance with clinical protocols, quality standards, and regulatory requirements

• Participate in interdisciplinary case reviews and quality improvement initiatives

Requirements

• Active Nurse Practitioner (NP) license in the required state(s)

• National NP certification (ANCC or AANP)

• Minimum 2+ years of clinical experience as an NP

• Experience in house calls, home health, geriatrics, primary care, or internal medicine (strongly preferred)

• Prior telehealth or remote care experience (preferred)

• Strong clinical judgment and comfort managing complex patients remotely

• Excellent communication and documentation skills

• Must be licensed in NY and one of the other states listed, or willing to obtain

• Active DEA registration (or willingness to obtain, if applicable)
