Description

Provide primary medical care services to nursing home patients in collaboration with the primary healthcare team. You will evaluate patients, order diagnostic tests, recommend treatment options, prescribe medications, and maintain high-quality clinical documentation and safety standards.

Responsibilities

• Provide primary medical care to nursing home patients within the standard of care

• Order laboratory and radiology tests; refer to specialists when appropriate

• Analyze reports, test results, medical records, and examinations to diagnose conditions and propose treatment options

• Prescribe pharmaceuticals and other treatments to address documented conditions

• Discuss potential side effects with patients; ensure allergy information is current before prescribing

• Provide lifestyle and diet guidance to improve health and manage conditions

• Follow departmental policies, quality improvement objectives, and safety/infection-control standards

• Maintain patient confidentiality and comply with federal and state health information privacy laws

• Record complete, timely, and legible medical records

Requirements

• BC/BE in Geriatrics, Internal Medicine, Palliative Care/Hospice, or Family Medicine

• MD or DO degree from an accredited United States medical school

• Successful completion of residency (US residency or 5-year pathway)

• Active NY license / current New York State Medical Practitioner License

• Current DEA and Department of Public Safety (DPS) registration for prescription writing (may be obtained during credentialing)

• Experience providing primary care and diagnostic services in a clinical setting

• Experience working in a team of healthcare professionals and collaborating to support positive patient experiences
