Description

Provide advanced primary and/or specialty care to patients. Perform physical exams, record medical history, develop care plans, order and interpret diagnostic tests, prescribe treatments, and administer vaccinations. Consult specialists and refer patients within the network when needed.

Responsibilities

• Examine, diagnose, and treat patients in general practice
• Ensure delivery of quality care
• Collect, record, and maintain accurate patient information
• Document past medical, family, and social history; review of systems; examinations; medications; allergies; assessment, and plan
• Order or execute tests, analyses, and diagnostic imaging
• Analyze test and exam reports to diagnose conditions
• Administer or prescribe treatments
• Order lab and imaging tests to guide immediate treatment plans
• Vaccinate patients to prevent communicable diseases
• Advise patients on diet, hygiene, and disease prevention
• Consult with medical specialists when necessary and refer patients within the network

Requirements

• Master’s Degree in a Nurse Practitioner Program (Family or Adult Geriatrics)
• Current New York State License as a Nurse Practitioner
• National Board Certification (AANP, ANCC, or ONCC)
• Bilingual Spanish speaking is a plus
• EMR/EHR experience is a plus
