Description

Provide advanced primary and/or specialty care to patients. Perform physical exams, record patient medical history, create care plans, administer diagnostic tests, and document assessments and plans. Diagnose and treat a variety of diseases in general practice while coordinating with specialists when needed.

Responsibilities

• Examine, diagnose, and treat patients
• Ensure quality of care aligned with the organization’s commitment
• Collect, record, and maintain accurate patient information
• Document medical history (past medical, family, social), review of systems, examinations, medications, allergies, assessment, and plan
• Order or execute tests, analyses, and diagnostic images; interpret results to diagnose conditions
• Administer or prescribe treatments
• Order lab and imaging tests to support an immediate treatment plan
• Vaccinate patients to immunize against communicable diseases
• Advise patients on diet, hygiene, and methods to prevent disease
• Consult appropriate medical specialists when necessary and refer within the network

Requirements

• Master’s Degree in a Nurse Practitioner program (Family or Adult Geriatrics)
• Current New York State Nurse Practitioner license
• National Board Certification via AANP, ANCC, or ONCC
• Bilingual (Spanish speaking) is a plus
• EMR/EHR experience is a plus
• New graduates are encouraged to apply

Benefits

• Salary: $135,000–$150,000
• Salary may reflect bonuses/incentives
