Description

Provide outpatient primary care for acute illness/injury and chronic conditions such as diabetes, hypertension, high cholesterol, men’s and women’s health issues, mental health concerns, and skin concerns. The clinic’s electronic health record system is AthenaHealth, and optional services may include office-based procedures and addiction medicine support through affiliates. Telehealth consultations in Colorado are available, and you can choose to use the telehealth platform.

Responsibilities

• Deliver outpatient primary care visits across acute and chronic presentations
• Manage common chronic diseases and everyday primary care needs
• Optionally provide office-based procedures (based on your preference)
• Optionally offer addiction medicine services (some delivered via affiliates)
• Optionally provide services through telehealth in Colorado
