• Full-time Family Medicine Physician role in post-acute care

Description

Provide high-quality post-acute care to skilled nursing facility patients in the Orlando area. The role includes rounding with NP support, managing patient care using the nursing home EMR for charting, and caring for short-term skilled patients with 3 visits per week. Day on-call coverage runs from 8 AM to 6 PM for the facilities rounded that day.

Responsibilities

• Perform Monday–Friday rounding on 1–2 buildings per day depending on census
• Use NP support to assist with patient care
• Manage 20–22 patients per day
• Chart in the nursing home EMR
• Provide comprehensive care to skilled, short-term patients with 3 visits per week
• Attend to 3–5 buildings as needed
• Provide day call coverage 8 AM–6 PM for facilities rounded that day
