Description

Provides biopsychosocial assessment, crisis intervention, short-term counseling, psycho-education, and linkage to resources for patients and families across care settings. Collaborates with the interdisciplinary treatment team on discharge and transitions of care, facilitates goals-of-care conversations, and may facilitate support groups. Practices independently and provides psychosocial consultation and community program liaison services.

Responsibilities

• Conduct biopsychosocial assessments and provide clinical interventions

• Deliver crisis intervention and short-term counseling to patients and families

• Provide psycho-education and facilitate support groups

• Support adjustment to illness and participate in goals-of-care conversations

• Consult on psychosocial aspects of care as part of the interdisciplinary treatment team

• Liaison with community programs and support discharge planning and transitions of care

• Educate staff/other hospital departments and participate on committees as needed

Requirements

• Master’s degree in Social Work (or related field)

• Active LCSW licensure (Licensed Clinical Social Worker)

• Departmental credentialing completed within 45 days of hire prior to billing

• 2 years recent relevant experience

• Outpatient: CCS (California Children Service) paneled

• Home Health: AUTO (automobile insurance) paneled/required

• Home Health: DL (valid driver’s license) required
