Description

Provides nursing care in patients’ homes under direct RN/physician supervision, following legal scope of practice, standards of care, and established policies and procedures. Implements the patient’s plan of care based on the diagnosis in a timely manner, supports patient and family education and communication, and helps keep the department prepared through supply distribution and equipment maintenance/testing/setup. Ensures privacy and safety by adhering to all applicable local/state/federal regulations and codes.

Responsibilities

• Deliver nursing care within LVN scope under RN/physician supervision
• Implement the patient’s plan of care based on diagnosis
• Support patient interaction, education, and cooperation (patient, family/support group, other providers)
• Distribute supplies and perform equipment maintenance, testing, and setup as needed
• Interpret data and instructions and exercise sound clinical judgment
• Follow privacy, safety, and all applicable regulations and policies

Requirements

• Graduate of an accredited Vocational Nursing program (prior to start date)
• LVN license provided prior to start date
• BLS Healthcare Provider certification
• Blood draw certificate within 30 days
• Valid driver’s license (not required for AIM Population Health)
• Auto insurance (not required for AIM Population Health)
