Description

Provide assessment, coordination, delivery, and evaluation of nursing care, including direct patient care, patient/family education, and transitions of care in an acute setting.

Deliver holistic and individualized care using a plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors.

Maintain collaboration with patients, families, physicians, and healthcare providers through timely communication while following hospital policies, industry standards, best practices, and applicable laws and regulations.

Responsibilities

• Assess and develop, implement, and coordinate a nursing plan of care

• Provide direct patient care and educate patients and families

• Coordinate transitions of care

• Communicate effectively with patients, families, physicians, and other providers

• Use critical thinking to identify and solve routine or complex problems

• Work independently and collaboratively in a fast-changing environment

• Manage priorities and assignments according to policies and time-sensitive needs

• Ensure privacy and protection of each patient’s PHI

Requirements

• Graduate of an accredited school of nursing OR RN license demonstrating completion of an RN program

• RN license for the state where care is provided

• BLS (Basic Life Support) Healthcare Provider certification

• ACLS (Advanced Cardiac Life Support) certification (department required)

• NIHSS (National Institutes of Health Stroke Scale) certification (department required)

• 2 years of recent relevant experience
