Description

The objective of this staff physician position is to establish a comprehensive, community-based, family-centered primary care program. The program is designed to improve access to high-quality primary care and improve health outcomes through prevention, health education, and evidence-based chronic disease management.

The physician will develop individualized care plans, lead preventive care and immunizations, and coordinate referrals and ongoing patient management.

Responsibilities

• Develop a care plan for each patient (history, physical exam, diagnosis, treatment and/or referral)

• Use available resources such as laboratory and radiologic testing to diagnose and confirm/deny disease

• Provide patient care without discrimination by age, race, color, national origin, ethnicity, sex, sexual preference, weight, handicap, or income

• Immunize and vaccinate patients per current standard of care

• Confer with consulting physicians, nurses, patients, and families while maintaining confidentiality

• Refer cases requiring specialist services in alignment with managed care contracts and approved provider networks

• Participate in practice development, quality assurance, peer review, and staff meetings/activities as needed

• Collaborate with other providers and support staff to support effective communication and continuous improvement of the site’s clinical operation

• Maintain accurate, timely documentation of all patient encounters per CHC/payer/federal/accreditation requirements

• Provide services to a full patient panel (minimum 1,500 active patients per provider FTE; “active” defined as at least one completed encounter in the past two years)

• Meet Federal Provider Productivity Initiative Review (PPIR) standards (e.g., minimum 4,200 encounters per fiscal year for full-time physicians and cost-per-encounter compliance under the federal threshold)

• Provide coordinated, comprehensive care across major life cycles and relevant settings (ambulatory outpatient, consultation, community medicine, mentoring/supervision, and hospital-required services)
