Description

Join as an Internal or Family Medicine trained physician to deliver full-scope primary care, including diagnosis, treatment, coordination of care, preventive care, and health maintenance. The role emphasizes high-quality clinical diagnostic skills, chronic condition management, and a patient-centered experience with an interest in Population Health Management. You will review patient history and test results, prescribe medications, maintain accurate medical records, and collaborate with the clinical team to ensure quality care.

Responsibilities

• Review patient history and perform physical examinations
• Evaluate, diagnose, and provide appropriate treatment and patient care; refer to specialists as needed
• Review test results and recommend suitable management plans, including preventive health recommendations
• Prescribe medications and formulate ongoing treatment and disease management plans
• Provide relationship-centered care considering patients’ psychosocial and physical needs
• Collect and record patient information (medical histories, reports, examination findings)
• Maintain complete and accurate medical records
• Communicate effectively with the clinical team to ensure delivery of quality care
• Participate in medical staff educational programs and meetings

Requirements

• Minimum 2–3 years of primary care experience in an outpatient setting
• Ability to diagnose and treat a wide variety of chronic conditions
• Demonstrated competency in diagnosis and treatment to maintain patient care and safety standards
• Patient-centered approach and excellent communication skills
• Interest in Population Health Management
• MD or DO (or foreign equivalent)
• Three years of post-graduate training from a U.S.-accredited residency in Internal Medicine or Family Medicine
• Board Eligible/Board Certified in Internal Medicine or Family Medicine
• Current, unrestricted New York State medical license
• Active DEA registration (or eligibility to register)
• Active Medicare/Medicaid enrollment (or eligibility to enroll)
• BLS or ACLS required
• Comfortable with Electronic Medical Record (EMR) systems and Microsoft Office
