Description

The Physical Therapist evaluates assigned patients’ physical therapy needs and implements a comprehensive, medically prescribed rehabilitation program under supervision. The role includes establishing and administering individualized treatment programs with realistic, achievable goals aligned to patient capacity and tolerance.

Responsibilities

• Document and record patient condition and educate patients and families on appropriate physical therapy methods
• Perform initial assessment and evaluation for specifically assigned patient populations
• Review medical charts to identify important medical information and clarify referrals
• Select appropriate assessment tools and methods to determine patient status and develop the optimal treatment plan (including treatment orders if needed for unfamiliar diagnoses)
• Develop and implement discharge plans with the patient, care partners, and the broader care team

Requirements

• Bachelor’s Degree in Physical Therapy
• New York State Physical Therapy License
• Good communication and interpersonal skills
• 0–1 years’ experience as a Physical Therapist
• Bilingual Spanish fluency strongly preferred
