About the Job

The CalAIM program provides services to participants receiving services through managed care plans. Staff support eligible Medi-Cal beneficiaries experiencing homelessness, including those with complex physical, behavioral, or developmental health needs, with the goal of improving health outcomes and reducing high-cost service utilization. The Case Manager assists clients in breaking the cycle of homelessness using a "whatever it takes" approach.

Responsibilities

  • Conduct intake and individualized needs assessments and develop Individualized Service Plans (ISP)
  • Address housing, income, medical, mental health, substance use, vocational, financial, and social support needs
  • Provide referrals, linkages, information, and ongoing support
  • Complete program-specific assessments (e.g., VI-SPDAT)
  • Evaluate strengths and barriers; conduct 90-day reviews after intake
  • Maintain accurate, timely documentation and complete participant files

Preferred Qualifications

  • Two (2) years of experience in healthcare, case management, homeless services, or related field
