Description

As part of the Interim Housing Services team, the Case Manager's main function is to assist clients in breaking the cycle of homelessness by utilizing a "whatever it takes" approach. This position supports clients through all phases of homelessness, including on the street, interim housing, transitional housing, and permanent housing.

The role includes assisting clients with accessing and maintaining necessary services among healthcare, social services, interpersonal support systems, and navigating through a coordinated entry system (CES). The Case Manager provides individualized supportive services by helping each client develop a plan to address strengths, barriers, support health needs, increase income, and find and maintain housing.

Responsibilities

  • Conduct intake and individualized needs assessments and develop Individualized Service Plans (ISP)
  • Provide referrals, linkages, information, and support to achieve ISP goals
  • Complete program specific assessments (e.g. VI-SPDAT)
  • Conduct 90-day reviews and evaluate short- and long-term goals
  • Maintain timely and accurate documentation and participant files
  • Ensure confidentiality per HIPAA guidelines
  • Develop therapeutic and professional relationships to support housing goals
  • Provide crisis intervention and risk assessment services
  • Use motivational interviewing and trauma-informed care in service delivery
  • Maintain knowledge of community resources and provide wrap-around services

Qualifications

Preferred: Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations.

Minimum: All levels of education and experience are welcome.
