About the Job

The Case Manager role's main function is to assist clients in breaking the cycle of homelessness by utilizing a "whatever it takes" approach. This position supports clients through all phases of homelessness, including but not limited to: on the street, interim housing, transitional housing, and permanent housing. They assist clients with accessing and maintaining necessary services among healthcare, social services, interpersonal support systems, and navigating through a coordinated entry system (CES).

Responsibilities

  • Conduct intake and individualized needs assessments for participants
  • Develop Individualized Service Plans (ISP)
  • Support housing, income, medical, mental health, substance use, vocational, and social support goals
  • Provide referrals, linkages, information, and support services
  • Complete program-specific assessments such as VI-SPDAT
  • Evaluate participant strengths and challenges
  • Conduct 90-day participant reviews
