Description

Provide comprehensive primary care to an expanding adult patient community. Work closely with the Practice Lead to ensure continuity of care across preventive visits, acute sick visits, post-hospital discharge follow-ups, pre-operative clearances, and chronic disease management. Use structured appointment slots in AthenaHealth EMR with limited telehealth.

Responsibilities

• Deliver full-spectrum adult outpatient medicine
• Conduct preventive wellness exams and Annual Wellness Visits
• Provide acute sick visits and routine follow-ups
• Manage post-hospital discharge follow-ups
• Perform pre-operative clearances
• Support chronic disease management; women’s health/well-woman exams optional
• Work within structured appointment slots (30-minute for new patients/AWVs/pre-ops/discharges; 15-minute for follow-ups)

Requirements

• Board Certified Nurse Practitioner (AANP or ANCC) in Adult, Adult-Gerontology, or Family Primary Care
• Active, unrestricted APRN license in Maryland with prescriptive authority
• 1+ years of outpatient primary care or internal medicine experience preferred (immediate integration-ready NPs welcome)
