Description

You will provide healthcare services that meet state and national standards by assessing and diagnosing patients, ordering appropriate lab and radiology tests, and coordinating referrals. You will prescribe medications and treatment regimens, counsel patients on side effects and lifestyle/diet changes, and support quality improvement and safety standards. You will work closely with other healthcare professionals to deliver high-quality patient care.

Responsibilities

• Provide primary medical care to nursing home patients in the standard of care

• Order laboratory and radiology tests and refer patients to specialists when appropriate

• Analyze reports, test results, medical records, and examinations to diagnose conditions and propose treatment options

• Prescribe pharmaceuticals, medications, and treatment regimens for documented medical conditions

• Review allergies and discuss potential medication/treatment side effects with patients

• Provide advice on lifestyle and diet changes to improve health and treat conditions

• Follow departmental policies, procedures, quality improvement objectives, and safety/infection control standards

• Maintain patient confidentiality and comply with federal/state health information privacy laws

• Record complete, timely, and legible medical records

Requirements

• BC/BE in Geriatrics, Internal Medicine, Palliative Care/Hospice, or Family Medicine (and Board Certified/Board Eligible for specialty openings)

• MD or DO from an accredited U.S. medical school

• Completed residency (or approved U.S. residency/5-year pathway)

• Active NY license / Current New York State Medical Practitioner License

• Current DEA and DPS registration to prescribe (or ability to obtain during credentialing)

• Experience providing primary care and diagnostic services in a clinical setting

• Experience working in teams and collaborating with healthcare professionals
