About the Role
The Clinical Health Advocate (Certified Medical Assistant) is an integral part of the care team, owning patient relationships and supporting a collaborative care plan that drives meaningful impact in patients' lives.
This is a part-time (up to 24 hours/week) hybrid role, including field visits within Aroostook County, work from an office in Presque Isle, and some remote work from home. The role is 50% clinical and 50% administrative, with general work hours scheduled Monday through Friday from 8am–5pm ET.
Responsibilities
Clinical
- Obtain and document vital signs (blood pressure, temperature, pulse, respirations, weight, height) in the EHR
- Perform venipuncture and capillary blood collection following proper phlebotomy protocols
- Collect, process, and prepare specimens for laboratory analysis
- Administer immunizations and injections per physician orders and maintain accurate vaccine records
- Assist providers with minor procedures and examinations
- Complete physical and social screenings as assigned and relay relevant environmental information to the care team
- Educate patients on treatment plans, medications, and follow-up care
- Document patient interactions, clinical data, and care activities in the electronic health record
- Support population health care gap closure efforts per payer contracts
- Ensure sterilization and maintenance of medical equipment
- Follow infection control protocols and safety standards
- Support field and urgent deployments as needed
- Connect patients with clinical and administrative services
Administrative
- Manage empaneled patient charts, ensuring medical and demographic readiness
- Partner with the care team to ensure orders and care coordination are completed
- Maintain cleanliness and organization of exam rooms, mobile units, and office spaces
- Manage clinical supply and medication inventory
- Route inbound mail and send patient letters or medical equipment
- Coordinate prior authorizations and referrals with insurance providers
- Request, follow up, and file medical records with hospital systems and providers
- Submit population health documentation to support payer contract compliance
- Communicate with patients via text, video, or phone for care coordination and scheduling
- Participate in projects and events as assigned
Qualifications
- At least 2 years of clinical experience as a Licensed Practical Nurse or Certified Medical Assistant
- Phlebotomy experience required
- Experience with injections (vaccines) preferred
- Reliable access to personal transportation
- Ability to manage multiple priorities and maintain strong organization
Compensation
- $27 per hour
