About the Role

The Clinical Health Advocate (Certified Medical Assistant) is an integral part of the care team, owning patient relationships and quarterbacking a collaborative care plan that drives meaningful impact in patients' lives. This is a part-time (up to 24 hours/week) hybrid role, including field visits within Aroostook County, work from an office based in Presque Isle, and some remote work from home. The role is 50% clinical and 50% administrative, with general work hours scheduled Monday through Friday from 8am–5pm ET.

Clinical Responsibilities

  • Obtain and document vital signs (blood pressure, temperature, pulse, respirations, weight, height) in the EHR
  • Perform venipuncture and capillary blood collection for laboratory testing following proper phlebotomy protocols
  • Collect, process, and prepare specimens for laboratory analysis
  • Administer immunizations and injections (as able) per physician orders and maintain accurate vaccine records
  • Assist providers with minor procedures and examinations
  • Relay environmental factors impacting patient well-being to the care team and complete physical and social screenings as assigned
  • Educate patients on treatment plans, medications, and follow-up care as directed
  • Document patient interactions, clinical data, and care activities accurately in the electronic health record
  • Carry out population health care gap closure efforts per payer contracts
  • Ensure proper sterilization and maintenance of medical equipment
  • Follow infection control protocols and maintain safety compliance
  • Be available for field and urgent deployment within the market region
  • Connect patients with clinical and administrative services

  • Represent the organization professionally and build relationships with stakeholders
  • Manage empaneled patient charts, ensuring medical and demographic readiness
  • Partner with the care team to ensure seamless order management and care coordination
  • Maintain cleanliness and organization of exam rooms, mobile units, and office spaces
  • Manage inventory of clinical supplies and medications
  • Route inbound mail and send patient letters or medical equipment
  • Coordinate with insurers for prior authorizations and referrals
  • Request, follow up, and file medical records with hospitals, PCPs, and HIEs
  • Submit population health documents to support payer contract compliance
  • Communicate with patients via text, video, or phone for coordination and scheduling
  • Participate in projects and events as assigned

Requirements

  • At least 2 years of clinical experience as a Licensed Practical Nurse or Certified Medical Assistant
  • Experience with phlebotomy is required
  • Experience with injections (vaccines) preferred
  • Reliable access to personal transportation
  • Strong organizational and communication skills
  • Ability to manage multiple priorities effectively

Compensation

  • $27 per hour
