About the Role

The Clinical Health Advocate (Certified Medical Assistant) is an integral part of the care team, owning patient relationships and coordinating a collaborative care plan that drives meaningful impact in patients' lives. This is a part-time (up to 24 hours/week) hybrid role with field travel within Aroostook County, office work based in Presque Isle, and some remote work from home. The role is 50% clinical and 50% administrative, scheduled Monday through Friday from 8am–5pm ET.

Clinical Responsibilities

  • Obtain and document vital signs (blood pressure, temperature, pulse, respirations, weight, height) in the EHR during patient visits
  • Perform venipuncture and capillary blood collection following proper phlebotomy protocols
  • Collect, process, and prepare specimens for laboratory analysis
  • Administer immunizations and injections per physician orders and maintain accurate vaccine records
  • Assist providers with minor procedures and examinations
  • Conduct physical and social screenings as assigned and relay relevant environmental factors impacting patient well-being
  • Educate patients on treatment plans, medications, and follow-up care as directed
  • Accurately document patient interactions and clinical data in the electronic health record
  • Support population health initiatives and care gap closure efforts per payer contracts
  • Ensure sterilization and maintenance of medical equipment
  • Follow infection control and safety protocols
  • Be available for field and urgent deployment within the market region
  • Connect patients with clinical and administrative services

  • Partner with the care team to coordinate orders and ensure seamless communication across care plans
  • Maintain cleanliness and supply levels of exam rooms, mobile units, and office spaces

  • Request, follow up, and file medical records with hospitals, PCPs, and HIEs
  • Submit population health documentation for payer compliance
  • Communicate with patients via text, video, or phone for care coordination and scheduling
  • Participate in projects and events as assigned

Qualifications

  • At least 2 years of clinical experience as a Licensed Practical Nurse or Certified Medical Assistant
  • Experience with phlebotomy is required
  • Experience with injections (vaccines) preferred
  • Reliable access to personal transportation

Compensation

  • $27 per hour
