Position Summary

Blackbird Health's Care Navigators are an essential part of our integrated and collaborative care team. Once a new patient or family has begun care, you serve as a constant source of support and operate as the owner of their experience—answering questions, providing education, guiding next steps, and ensuring each patient is well cared for.

How You'll Make an Impact

Deliver a Seamless Patient Experience:

  • Serve as the main point of contact for established patients
  • Field inbound calls, messages, and emails with responsiveness and professionalism
  • Answer questions, track down information, and ensure transparent follow-up

Navigate the Care Journey:

  • Guide patients and families through the care model
  • Schedule and confirm appointments, including evaluations and ongoing therapy or medication checks
  • Manage a specific group of providers and ensure appropriate scheduling
  • Manage provider orders, screenings, and scheduling tasks

In-Office Support:

  • Maintain a welcoming and professional in-office environment
  • Support patients and providers in person
  • Manage mail, restock supplies, and handle office opening and closing duties

Uphold Privacy & Compliance:

  • Adhere to strict confidentiality and data protection standards
  • Ensure compliance with HIPAA regulations and family consent laws

This position is onsite, 5 days a week.
