Description

Provide compassionate, home-based hospice and palliative care through face-to-face visits, assessments, symptom management, and individualized care plan development. You will coordinate with physicians and an interdisciplinary team, participate in goals-of-care discussions, and support patients and families through serious and life-limiting illnesses.

Responsibilities

• Conduct face-to-face hospice and palliative care visits in patients’ homes
• Perform comprehensive assessments and manage symptoms
• Develop and update individualized care plans
• Participate in goals-of-care discussions and advance care planning
• Assist with POLST and advance directive discussions
• Provide education and support to patients and families
• Coordinate care with physicians, community providers, and the interdisciplinary team
• Participate in interdisciplinary team meetings and quality initiatives
• Maintain timely and accurate clinical documentation

Requirements

• Active California NP and RN licenses
• DEA registration
• NPI
• MSN degree
• AANP or ANCC certification
• BLS certification
• 3+ years of hospice or palliative care experience
• ACHPN certification preferred at hire and required within one year
• Valid California driver’s license and appropriate vehicle insurance
• Comfortable providing independent home-based care
• Spanish bilingual skills are a plus
