Description

The incumbent provides advanced professional nursing case management and care coordination for eligible American Indian and Alaska Native beneficiaries. The role manages complex, precedent-setting cases independently, provides clinical leadership to lower-graded nursing staff, and leads quality improvement efforts. Work includes culturally informed assessments and coordination across providers, disciplines, and care settings.

Responsibilities

• Conduct nursing assessments incorporating medical history, current clinical status, psychosocial factors, cultural considerations, and social determinants of health
• Monitor patient progress via follow-up activities, review of clinical data, and participation in case conferences
• Coordinate care across providers, disciplines, and care settings to ensure continuity and prevent service fragmentation
• Provide direct professional nursing care to assigned patients with chronic and recurring health conditions

Requirements

• U.S. citizenship
• Selective Service Registration for males born after 12/31/1959
• Successful completion of a federal background investigation
• Financial suitability (demonstrate financial responsibility; no substantial delinquent or unresolved financial obligations)
• Pass E-Verify employment verification
• Education from an accredited professional nursing program (per listed accrediting bodies) OR required foreign nursing certification
• Passed National Council Licensure Examination (NCLEX)
• Current, active, full, unrestricted RN license/registration
