Description

An experienced RN responsible for overall management of patients in an assigned caseload, including direct care delivered to meet designated visit productivity standards. Plans, organizes, and directs patient care services in accordance with current standards and regulations, and coordinates the interdisciplinary team to implement the patient care plan. Ensures effective management of visit utilization and helps control expenses; may support orientation, competency evaluations, and consultation to clinical staff.

Requirements

• Graduate of an accredited School of Nursing
• RN license in California
• BLS (Healthcare Provider) certification
• Valid driver’s license
• Automobile insurance and reliable transportation
• 2 years recent relevant experience
• Independently functional and able to work as part of an interdisciplinary team
• Demonstrated clinical assessment and independent nursing judgment
• Knowledge of pathophysiology and accepted treatment protocols for common home care diagnoses
• Knowledge of safety and infection control principles
• Recognize special needs of Hospice patients and others in the home
• Ability to communicate effectively and professionally with patients and significant others
• Understanding of palliative care for terminally ill patients
• Recognize unsafe or emergency situations and act appropriately
• Meet productivity standards; solve unique problems or consult supervisor as needed
• Maintain required licensure and certifications
