Nurse Practitioner / Physician Assistant (Primary Care)

• Role: Board-Certified Advanced Practice Provider (Nurse Practitioner or Physician Assistant)

• Setting: Federally Qualified Health Center (FQHC) primary care team (PCMH model)

• Schedule: Outpatient with standard daytime hours plus 1 evening shift per week; no hospital rounding

• Call: Light, shared, strictly phone-based after hours with a triage answering service

• Team support: Dedicated MA, RN/LPN, and Integrated Behavioral Health Consultant

• Practice size: 8–12 primary care providers plus supporting dental and LCSW staff

• Patient volume: ~16–22 patients/day (pediatric to geriatric)

• Compensation: $125k–$135k+ (DOE)

• Loan repayment: HRSA/NHSC federal student loan repayment eligibility due to FQHC status

Role Description

Join an established primary care team at an FQHC operating within a Patient-Centered Medical Home (PCMH) model, supported by a dedicated clinical staff and integrated behavioral health resources.

Responsibilities

• Provide outpatient primary care to pediatric through geriatric patients

• Participate in a shared after-hours on-call schedule that is strictly phone-based

• Work collaboratively with MA, RN/LPN, and the Integrated Behavioral Health Consultant as part of the PCMH care team

Requirements

• Board certification as a Nurse Practitioner or Physician Assistant

• Experience: 3+ years preferred; recent graduates strongly encouraged

• HRSA/NHSC eligibility for federal student loan repayment via the FQHC status
