Description

Provide integrated primary care in a patient-centered medical home (PCMH) model, focusing on annual wellness visits, health assessments for patients with chronic conditions, and patient health education. Collaborate with physicians, mid-level providers, pediatric teams, dental providers, behavioral health, and clinical staff to close quality care gaps such as screenings and labs. Requirements

• NP or PA licensure to practice
