Description

Provide integrated, patient-centered primary care in a patient-centered medical home (PCMH) model, focusing on annual wellness visits and health assessments for a population with chronic conditions. You’ll support patient education and help close quality care gaps such as screenings and labs, while collaborating with a multidisciplinary team.

Responsibilities

• Perform annual wellness visits and health assessments
• Provide patient health education opportunities
• Assist in closing quality care gaps (screenings and labs)
• Act as an enthusiastic collaborator to enhance care delivery

Requirements

• Active licensure as an NP or PA
• Experience in primary care and caring for patients with chronic conditions
• Ability to perform annual wellness visits and health assessments
