Description

Federally Qualified Health Center (FQHC) seeks a Board-Certified Advanced Practice Provider (NP or PA) to join an established primary care team. The position is entirely outpatient with standard daytime hours and only one evening shift per week, plus light phone-only shared call after hours. You will practice within a PCMH model with support from clinical staff and integrated behavioral health resources.

Responsibilities

• Provide outpatient primary care for pediatric to geriatric patients

• Work within the Patient-Centered Medical Home (PCMH) model

• Coordinate care with a dedicated care team (MA, RN/LPN, Integrated Behavioral Health Consultant)

• Participate in light phone-based shared call after hours using a triage answering service
