Description

Join an outpatient primary care practice serving the community. You will provide care at each visit and manage assessment and plans, including thorough EHR documentation, review of prior results, prescriptions, and follow-up.

Responsibilities

• Practice medical care for each patient visit
• Complete pertinent history and physical exam
• Document chief complaint, history of present illness, pertinent review of systems, and physical exam in the EHR
• Review and update past medical history, family history, social history, medications, and allergies in the EHR
• Review vital signs
• Review results and reports from prior orders; place new orders
• Perform and document procedures
• Administer medications and immunizations
• Determine and document assessment and plan
• Prescribe medications
• Provide patient education, guidance, and counseling; arrange follow-up
• Sign EHR encounters within established timelines per policy
• Respond to patient questions or needs arising after the encounter
• Adhere to established scheduling templates
• Participate in quality metrics directives and departmental quality improvement (QI) projects
• Attend practitioners’, clinical site, and practice-wide meetings
• Read emails and other organizational communication platforms
• Complete peer review
• Comply with Clinical Quality Measures expectations
• Use resources to verify standards of care and modify workflows to reach QI objectives
• Consistently document activities related to QI projects
