Job Summary

We are seeking a dedicated and compassionate QM Specialty NP / PA to join our facility in the Bronx, NY. As a QM Specialty Provider, you will provide comprehensive primary care services via telehealth to a diverse patient population. This is a part-time position ideal for a provider who values meaningful patient relationships and a collaborative care environment.

Responsibilities

  • Conduct assessments, diagnose, and develop treatment plans tailored to patient needs
  • Manage chronic diseases and acute conditions
  • Collaborate with interdisciplinary teams to enhance patient care
  • Maintain accurate and timely medical records in compliance with documentation standards

Requirements

  • Ability to collaborate with interdisciplinary teams and external stakeholders
  • Strong clinical judgment, communication, and patient engagement skills
  • Proficiency with electronic medical records (EMR) and telehealth technology
  • Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
  • State-specific license in good standing in New York
  • Minimum 6 months of experience in long-term care or a skilled nursing facility
  • Strong communication, clinical, and critical thinking skills
