Key Responsibilities
The Triage Coordinator is responsible for the coordination of all after hours calls, after hours orders, and after-hours communication with on-call provider, patients, facilities, and families for Physician Services. Responsibilities are completed with limited supervision and after hours. This position requires initiative, creative problem resolution, time management, excellent phone and electronic communication skills, service excellence and a desire to work within a practice that centers around and with a geriatric population.
- Communicates significant findings, problems, changes in condition or environment to the primary care providers and other personnel involved with patient care
- Performs well under stress and in emergency situations
- Assumes primary responsibility for "after hours" including planning, coordinating, implementing, and assisting with the evaluation of the plan of care for patients, families and facilities
- Develops and maintains positive working relationships with others
- Maintains confidentiality of patient and employee information in accordance with policy
- Answers telephone calls after hours and responds to concerns or changes in patient condition
- Provides support and direction to agency staff, health care professionals, patients, and families related to health care concerns
Required Qualifications
- High School Diploma or GED
- Minimum one (1) year of experience in medical field, medical office experience or related position
- Ability to operate computer equipment, fax, and photocopy machines
- Proficiency in Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint
- Knowledge of medical terminology
- Excellent verbal and written communication skills
Desired Qualifications
- CNA or MA certification preferred
- Experience with geriatric population and/or caregivers preferred
- Familiarity with Ethizo EMR preferred
Hours
Four days per week with an every other weekend rotation.
- 3 week days, weekend, 3 week days, off weekend
- In office: 11 a.m. to 4 p.m. or 11 a.m. to 5 p.m.
- Lunch: 4 p.m. to 5 p.m. or 5 p.m. to 6 p.m.
- Remote: 5 p.m. to 10 p.m. or 6 p.m. to 10 p.m.
