Position Summary

The OneCare RN Manager provides clinical leadership, operational oversight, and program management for the OneCare Program—Correctional Healthcare Partners' integrated care model focused on chronic disease management, care coordination, and continuity of care for the incarcerated population.

This role combines direct nursing accountability with supervisory and administrative responsibilities, ensuring nursing staff deliver high-quality, patient-centered, and NCCHC-compliant care within a correctional healthcare environment. The OneCare RN Manager is responsible for staffing oversight, performance management, quality improvement initiatives, and collaboration with medical, mental health, and custody teams to support positive patient outcomes and operational excellence.

Essential Duties and Responsibilities

Program Leadership & Oversight

  • Lead the day-to-day operations of the OneCare Program, ensuring consistent delivery of chronic disease management, care coordination, and transitional care services.
  • Establish clinical and operational priorities aligned with organizational goals and NCCHC standards.
  • Monitor program performance metrics, patient outcomes, and quality indicators; provide regular reports and recommendations to leadership.
  • Drive Continuous Quality Improvement (CQI) initiatives by identifying opportunities for improvement, implementing corrective actions, and measuring outcomes.
  • Serve as the primary clinical liaison between nursing staff, medical providers, behavioral health staff, custody personnel, and facility leadership.

Staff Supervision & Development

  • Directly supervise Registered Nurses, Licensed Vocational Nurses, and Certified Nursing Assistants assigned to the OneCare Program.
  • Conduct performance evaluations, provide coaching and mentoring, and address performance concerns in collaboration with Human Resources.
  • Manage staffing schedules, coverage planning, and overtime utilization in accordance with operational and budgetary requirements.
  • Participate in recruitment, hiring, onboarding, orientation, and training activities.
  • Foster a culture of accountability, professionalism, teamwork, and patient-centered care.

Clinical Leadership

  • Maintain an active clinical presence and provide direct patient care when necessary to support staffing and operational needs.
  • Oversee nursing assessments, care plans, medication administration practices, and treatment implementation.
  • Ensure clinical documentation within the electronic health record (EHR) is accurate, timely, and compliant with NCCHC standards and organizational requirements.
  • Support and coordinate responses to medical emergencies, critical incidents, and urgent clinical situations.
  • Provide clinical guidance and support to nursing staff while promoting evidence-based nursing practice.

Compliance & Accreditation

  • Ensure compliance with NCCHC standards, HIPAA regulations, state nursing practice acts, and all applicable healthcare regulations.
  • Participate in accreditation surveys, internal audits, and regulatory reviews.
  • Develop, implement, and monitor corrective action plans when necessary.
  • Maintain current knowledge of correctional healthcare regulations, accreditation standards, and nursing best practices.

Qualifications

Education

  • Bachelor of Science in Nursing (BSN) required.
  • Master of Science in Nursing (MSN) preferred.

Licensure & Certifications

  • Active, unrestricted Registered Nurse (RN) license in the State of California.
  • Current BLS/CPR certification required.
  • ACLS certification preferred.

Experience
