About the Role
Practice nursing with autonomy, time, and meaningful patient connection. As a Home Health Registered Nurse, you will deliver comprehensive skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and overall health goals while helping individuals remain safely in their own environment.
Manage a caseload from admission through discharge, providing skilled nursing care, conducting assessments, and developing individualized care plans tailored to patient needs. Collaborate with physicians and an interdisciplinary care team to coordinate services, make informed clinical decisions, and shape patient outcomes.

Main Service Area Options:

This is a high travel position requiring daily travel within the service area to provide care in homes, assisted living facilities, or nursing homes.
Hours
Monday to Friday, 8 a.m. to 5 p.m., with weekend rotation.
Required Qualifications
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse
- Minimum 1 year of experience as a licensed RN in a clinical care setting
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Experience in home health environment
- OASIS certification or willingness to obtain

- Proficiency in OASIS documentation and care planning
- Excellent time management and organizational abilities

- Understanding of Medicare, Medicaid, and third-party payor requirements
Pay Range
$80,000—$87,000 USD
