Role Summary
As a Hospice RN Case Manager in the Dallas/Ft. Worth area, you will build meaningful relationships with patients and families while guiding care across the hospice journey. This role balances clinical expertise with thoughtful coordination, supporting comfort, dignity, and informed decision-making in the place patients call home.
Overview
The Hospice RN Case Manager provides skilled nursing assessment, planning, and care to hospice patients and their families. The RNCM manages an assigned caseload and collaborates closely with interdisciplinary team members to deliver compassionate, effective care aligned with patient goals and hospice regulations.
Key Responsibilities
- Complete initial and ongoing assessments of patient physical, emotional, psychosocial, and environmental needs
- Coordinate with physicians for certification and recertification of terminal illness in compliance with Medicare regulations
- Serve as the primary clinical contact for assigned patients, including care planning, symptom escalation, and advocacy
- Coordinate care with all providers and support systems to ensure seamless hospice services
- Develop and update individualized plans of care in collaboration with patients, families, and the interdisciplinary group
- Provide skilled nursing care in accordance with the plan of care and document interventions and outcomes promptly
- Ensure documentation supports regulatory, quality, and billing compliance requirements
- Educate patients and families on disease progression, symptom management, medications, and hospice philosophy
- Supervise hospice aides and support the development and revision of aide care plans
- Participate in interdisciplinary group meetings and coordinate with physicians, social workers, chaplains, and bereavement staff
- Communicate effectively with interdisciplinary team members and clinical leadership
- Participate in the on-call rotation and respond to urgent patient needs as required
- Escalate care needs to the medical director or covering provider as clinically indicated
- Perform other duties as assigned
Qualifications
- Graduate of an accredited nursing program; Associate's degree or higher preferred
- Current RN license in the state of practice or valid multistate compact license
- Minimum of one (1) year of hospice, palliative care, or home health nursing experience preferred
- CPR certification and valid driver's license required
- Strong clinical judgment, documentation accuracy, and communication skills
- Knowledge of hospice regulations and Medicare Conditions of Participation preferred
- Ability to work independently while collaborating across interdisciplinary teams

- $85,000 USD starting compensation

The Liaison collaborates closely with operational and clinical leaders to ensure alignment between referral needs and service capabilities. This is a performance-driven role focused on consistent outreach, education, and engagement across the assigned territory. Key Responsibilities - Represent the organization in the community with professionalism and compassion. - Develop and maintain strong relationships with physicians, hospitals, nursing homes, assisted living facilities, senior communities, and other referral partners. - Meet with patients and families to review hospice philosophy, explain services, answer questions, and obtain signed consents for care. - Educate healthcare providers and community members on hospice and palliative care services. - Conduct in-services, presentations, and community outreach events to raise awareness. - Support referral generation and facilitate timely communication with the intake and clinical teams. - Identify opportunities for partnership or collaboration to enhance access to care. - Track outreach activities and maintain accurate records in accordance with company documentation standards. - Maintain awareness of competitor activity and market trends in the assigned territory. - Participate in team meetings and contribute to ongoing strategy and planning discussions. - Support the mission to deliver high-quality, person-centered care. - Maintain confidentiality and adhere to all HIPAA regulations. - Perform other duties as assigned. Qualifications - Must currently reside in Broward County, Florida. - Bachelor's degree preferred. - Minimum of two (2) years of experience in healthcare sales, business development, or marketing; hospice or home health experience strongly preferred. - Strong interpersonal, communication, and relationship-building skills. - Ability to educate diverse audiences about hospice services with clarity and empathy. - Highly organized, self-directed, and goal-oriented. - Proficient with Microsoft Office and electronic medical record systems. - Valid driver's license and reliable transportation required. - Demonstrated empathy, sensitivity, and ability to support patients and families from diverse backgrounds. - Strong commitment to delivering high-quality, person-centered care aligned with mission, values, and standards of dignity, respect, compassion, and integrity. Compensation $75,000—$85,000 USD Final pay is determined based on factors such as experience, licensure, location, and business needs at the time of hire.
