Description

Provide patient-centered inpatient pharmaceutical care, including medication information, education, and medication preparation and distribution to ensure safe, effective, and cost-efficient medication therapy. Use clinical skills and judgment to contribute to and implement the patient’s plan of care based on the diagnosis, while communicating effectively with patients, families/support groups, and other healthcare providers. Ensure compliance with applicable regulations and protect patient privacy (PHI).

Responsibilities

• Deliver pharmaceutical care and medication-related patient services

• Provide medication information, education, and clinical support

• Assess and monitor patients and communicate recommendations in a timely manner

• Follow local/state/federal regulations, codes, and organizational policies and procedures

• Maintain privacy and safety for each patient’s PHI

• Perform department-specific tasks and may orient other staff

Requirements

• PharmD (graduation from an accredited school of Pharmacy)

• Licensed or registered pharmacist (PHARMD or PHARMR)

• ACLS certification if responding to codes

• 1 year of recent relevant experience
